Connexions


The service is able to support individuals from the age of 17 as part of a transitional model of support we promote at Regard. In each case we would work closely with all parties to ensure an agreed approach is put in place to help an individual make that transition in to adulthood a positive one.

Specialism: Connexions is a supported living service, consisting of 2 two-bedroom flats and 1 one-bedroom flat. The service has been designed to offer support and accommodation for adults with Autistic Spectrum conditions, behaviours considered to challenge, physical and mental health support needs and those with significant communication difficulties.

Type of Service: Supported living flats in the busy town of Redruth, Cornwall. Each flat has been refurbished to a high standard and decorated to reflect the individual’s tastes. Within easy walking distance of Redruth with its range of amenities, Connexions is well located to enable the tenants to access and participate fully in their community.

Age Range: adults 18 – 40

Current profile of the Service: Connexions provides supported accommodation for those moving from residential care, family home or educational establishments who would benefit from having their own accommodation; whilst being in close proximity to others if they wish.

Hours provided: There is 24 hour staffing, including waking night and sleep in staff, dependent on the identified needs of the tenants

Focus of the service: Connexions supports individuals with Autism, moderate to severe learning disabilities and complex needs, including behaviours considered to challenge and dual diagnosis. The service aims to provide each tenant with the opportunity to maximise their potential and to be valued and contributing members of the local community.

Connexions is a large building within the town centre that has been developed into two bedroom, self-contained flats. The property is being completely refurbished to a high standard throughout.

The first floor will have two 2 bedroom self-contained flats and on the ground floor there will also be two 2 bedroom self-contained flats. The development is being led by individual need on the ground floor.

Everyone living at Connexions will be actively involved in all decisions in relation to the running of their home and their personal life.

Individuals are involved in:

• Staff matching

• Quality checking and health and safety

• Changes to the living environment

• Development of service delivery

• Self-assessment through person-centred planning

• Integration with the local community, including support to take on part-time work, either paid or voluntary.

We have experience of supporting people with direct payments, self-directed support, personal budgets, individual service funds and privately funded individuals.

Leah Bone the service manager is qualified to NVQ Level 4 in Health & Social Care and NVQ Level 4 RMA, as well as holding a BSC Honours in Social Policy and Administration. She has 8 years management experience and has worked with Adults and Children with mild to severe learning difficulties and challenging behaviour since 1996. She regularly attends various training workshops to update and develop her skills.As well as this wealth of experience, staff are trained to cope with the needs (and changing needs) of the service-users, most recently having further additional training for medication, person-centred care planning, autism and risk assessments. The service is also supported by a certified instructor for behaviour management (Non-violent crisis intervention), provided through the Crisis Prevention Institute (BILD accredited).

Reason for nomination: Everyone living at Connexion Street has very complex support needs and traditionally, would have been unlikely to be considered for a supported living service.
