We support individuals with a variety of support requirements, ensuring they remain the centre of the support we offer. We involve the people that we support with the recruitment process to ensure they have a voice from day one around who supports them. Individuals are also encouraged to chose how they furnish and decorate their own home, as well as plan their daily life and long term goals and aspirations.

Each of the people we support is assigned a key worker, whom they become very familiar with. In addition, we encourage families to play a key role in the support we provide and ensure that they are involved to the degree the individual wishes them to be.

We work hard to provide excellent person-centred support.

We support individuals within their own homes, shared accommodation and individual premises to live as independent a life as possible and to assist individuals to develop in a variety of ways.

We support adults with learning difficulties, mental health needs, physical disabilities and other health care needs as required.

Jake has been working in the Health and Social Care sector since 2007. Through out this time has held a variety of roles supporting those with learning difficulties, physical disabilities and mental health needs. Jake ensures best practice by working closely with his Locality Manager, Service Managers, Team Leaders and the rest of the large domiciliary team.

The team it self has extensive knowledge and experience. Staff are regularly encouraged to improve their skills by completing training in order to to constantly improve the service that they offer.
